The Opportunity
As the registered Nurse, you will provide high quality clinical care to RACF residents, as well as leading the frontline care team to deliver safe and effective personal care and clinical care. Working closely with the facility's clinical leadership team, you will ensure our service & facility meet quality & compliance standards, and work to achieve best practice. Work with a skilled, collaborative team to prevent hospitalisation & maintain residents' quality of life.
The current opportunity is for a Casual and Part Time RNs, with rosters available across day, afternoon and night shifts.
Responsibilities:
- Ensuring care services are provided so each resident is treated with dignity and respect, with their identity, culture and diversity valued.
- Directly supervising the care team, including providing on-the-job coaching and feedback, allocating breaks & training, and ensuring clear & comprehensive handover communication
- Conducting head-to-toe assessments and responding to changes or deterioration in mental health, cognitive or physical function, capacity or condition.
- Providing a timely emergency response to consumers with acute deterioration, falls and infections which may be compounded by co-morbidities.
- Managing residents': pain; continence management; skin care & wound management; acute, complex or chronic diseases.
- Managing and delivering medications safely and competently, consistent with quality use of medicines guidelines.
- Implementing infection prevention, control, antimicrobial stewardship and outbreak management programs.
- Undertaking assessments, developing and updating care plans to meet prescribed timeframes, funding and consumer needs, goals and preferences.
- Participating in case conferences with residents and family members, and consulting with other health professionals in relation to complex clinical issues
About You
Essential Criteria
- Current unconditional Registered Nurse APHRA registration; with 3+ years' acute care experience and/or 5 years' aged care experience
- Demonstrated high level of clinical care and care management including assessment, care planning, monitoring and evaluation
- Committed to person-centred approach
- Demonstrated ability to successfully lead a frontline care team
- Highly effective written & verbal communication skills
- Compassion, reliability & integrity
- Functional IT skills & documentation experience
- Right to work in Australia
Desirable
- Knowledge and experience in the aged care industry and applicable funding instruments
- Experience with computerised clinical care management systems
