We are seeking a Recovery Support Worker to join our Annerley Hospital to Home team in a fixed-term, full-time role, supporting people at one of the most important transition points in their lives. You’ll work alongside people as they leave hospital and begin rebuilding life in the community, helping them regain confidence, reconnect with what matters to them and take practical steps towards greater independence and stability.
This is meaningful, hands-on work where no two days look exactly the same. You might be helping someone settle back into everyday routines, supporting them to find secure housing, reconnect with family or community, build practical skills or take the next step towards a personal goal they have been working towards. Sometimes the difference you make will be big and obvious. Other times, it will be the smaller moments: showing up, listening, helping someone feel heard, or backing them to do something they did not think they could do.
At the heart of the role is connection. You’ll build genuine, respectful relationships with the people you support and work alongside a team that believes good support starts with seeing the whole person, not just what has brought them into the service. You’ll also be joining a team that values collaboration, reflection and doing the work well. You’ll have the opportunity to use your judgement, build your skills and contribute to support that can genuinely change the direction of someone’s life.
Your Key Responsibilities:
- Provide practical, day-to-day support that helps clients work towards their individual goals, build everyday skills and strengthen independence.
- Deliver person-centred, recovery-oriented support grounded in compassion, empowerment, choice and mutual respect.
- Support people through important transitions, including hospital discharge, reconnecting with their community and accessing sustainable housing and other supports.
- Build positive relationships with clients and work collaboratively with families, community services, health providers and other stakeholders.
- Contribute to client assessments, risk planning and ongoing review to support safe and responsive service delivery.
- Maintain accurate and timely client records, documentation and reporting using organisational systems and mobile technology.
- Contribute to a positive team culture through supervision, reflective practice, feedback and continuous improvement.
You Will Have:
You’ll bring a practical, people-focused approach, with the confidence to work independently while building strong relationships with clients, colleagues and community partners.
- A Diploma in a human services related field and/or demonstrated experience supporting people experiencing mental health challenges.
- A genuine commitment to working alongside people in a respectful, strengths-based and person-centred way.
- The ability to work independently while remaining connected and accountable to your team.
- Strong organisation and time management skills, including the ability to manage a caseload, documentation and service requirements.
- Excellent communication and interpersonal skills, with the ability to build effective relationships with clients, colleagues and external services.
- Sound professional judgement, clear boundaries and a high level of integrity.
