About the position
The Disability Support Worker will provide direct support to people with disability and assist clients with daily living, personal care, community participation and other activities in accordance with individual support plans.
The successful applicant will work closely with clients and relevant healthcare and support professionals to provide safe, respectful and consistent support.
Key responsibilities
- Provide person-centred support to clients with disability.
- Assist clients with personal care and activities of daily living.
- Support clients with meal preparation, shopping and household tasks.
- Assist clients to attend appointments and participate in community activities.
- Encourage and support clients to maintain and develop their independence.
- Follow individualised care and support plans.
- Monitor and report changes in clients' health, wellbeing and behaviour.
- Maintain accurate client records, progress notes and other required documentation.
- Communicate appropriately with clients, families, carers and other service providers.
- Follow workplace health and safety, infection control, privacy and safeguarding requirements.
- Report incidents, risks and changes in client circumstances in accordance with organisational procedures.
- Work cooperatively with other members of the healthcare and support team.
