Your opportunity

As a Care Partner, you’ll be a trusted advisor and advocate for clients, helping them navigate services and funding to remain living safely and independently at home.

You’ll make an impact by

  • Developing and adapting personalised care plans that reflect each client’s goals, needs, and aspirations
  • Proactively identifying and responding to changing client needs in partnership with families and multidisciplinary teams
  • Delivering high-quality, client-centred care that supports positive outcomes and experiences
  • Managing client feedback and maintaining clear, timely communication with stakeholders
  • Supporting operational efficiency through accurate administration and coordination tasks

What you’ll bring

To succeed in this role, you’ll need the following qualifications:

  • Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields 
  • Experience in client case management 
  • National Police check  
  • Driver’s licence
