About the role

Wintringham’s Home Support team is seeking an experienced Care Partner to join our team. The role delivers person-centred care management and service coordination that supports older people to maintain their independence, wellbeing and quality of life while living in the community. Responsibilities include:

  • Support older people to live well by developing person-centred care plans that promote independence, wellbeing and quality of life.
  • Connect participants with the right services and supports, ensuring care is responsive, coordinated and aligned with funding requirements.
  • Build trusted relationships with participants, families, carers, service providers and community partners to achieve positive outcomes.
  • Respond to changing needs by monitoring wellbeing, identifying risks and escalating complex matters when needed.
  • Keep care on track by managing budgets, service agreements, records, compliance, feedback and complaints with care and accuracy.

What are we looking for?

  • Undergraduate and/or post graduate qualification in Community Services, Aged Care, Disability, Health, Nursing, Social Work or a related field. First Aid certificate - Level 2. Current valid Victorian Driver’s Licence.
  • Experience in care management, case management or service coordination within aged care, health, disability, homelessness or community services.
  • Ability to assess participant needs, develop care plans and coordinate services that support independence, wellbeing and quality of life.
  • Strong understanding of person-centred practice, participant rights, dignity of risk and informed decision making.
  • Skilled in identifying and responding to risks, changing needs and complex matters, including referrals and escalation.
  • Strong administration, communication and stakeholder engagement skills, with experience managing budgets, records, compliance, feedback and complaints.

What we offer

  • Employment type: Permanent part-time, 15.2 hours per week with flexibility to work over two or three days per week.
  • Pay: $40.52-$50.19 plus super (depending on your qualifications & experience). Above Award pay and not-for-profit salary packaging benefits of up to $18,500 each year.
  • Location: Shepparton, with travel in the Hume region.
