Helena Valley Medical Centre is an Accredited General practice with four full time GPs. We care for a diverse community, and our team is known for our great patient service. Our nurses are valued clinicians, not just support staff.About the role
We're looking for an experienced Practice Nurse to join our team on a part-time or casual basis 2-3 days per week. You'll be a key part of how we look after our patients, especially those living with chronic conditions. Your duties will include:-
Supporting patients with chronic disease management, including diabetes, respiratory conditions, heart disease, arthritis and mental health

Triaging patients

Clinical data management
-
Developing and coordinating care plans and Team Care Arrangements

ECG and Spirometry
-
Running health assessments, immunisation clinics and wound care

Working alongside our GPs and allied health providers, and connecting patients with local community services
Essential Criteria
-
Current AHPRA registration
-
Previous experience in general practice preferred

Strong knowledge of chronic disease management and care pathways

Excellent communication skills and a warm, patient-focused approach

The ability to prioritise, stay organised, and work both independently and as part of a team

Proven ability to work respectfully with people from diverse backgrounds and complex needs
Desirable Criteria

Accredited Immunisation Certificate

Asthma education and spirometry skills
-
Experience with Best Practice software
Previous general practice experience is highly regarded, but we are also open to nurses looking to transition into general practice.
