About the Facility
This role will be based at our Grandton Residence, supporting both our Home Care Services program and the broader community. The position plays a key role in ensuring the delivery of high-quality, person-centred care to clients living independently in their own homes.
This is a part-time opportunity, working Monday, Wednesday, and Friday 8am to 4pm with the potential to move to full-time once the client list has expanded.
About the Role
As a Home Care Coordinator, you will play a vital part in enhancing the lives of our clients by delivering personalised care plans aligned with their goals, preferences, and budgets.
What You Will Be Doing
- Assisting the Home Care Manager with coordinate and manage Home Care and Private Care services for clients
- Complete paperwork for admissions, manage the Home Care Roster and Home Care Billing
- Develop, implement, and review individualised care plans
- Schedule, roster, supervise, and support care staff to ensure service excellence
- Monitor client wellbeing, progress, and satisfaction
- Manage client budgets in line with Home Care Package guidelines
- Maintain accurate documentation and provide detailed reports to management
- Build strong relationships with clients, families, and healthcare professionals
- Ensure compliance with relevant legislation, policies, and standards
- Drive continuous improvement across care delivery and processes
- Address feedback and concerns promptly and professionally

Essential Requirements
- Experience in a coordination role within community aged care
- Certificate IV or Diploma in Individual Support or Similar
- Experience in rostering and billing in a Home Care setting
- Strong understanding of Home Care Packages and client-centred care
- Ability to work independently with excellent time management skills
- Outstanding communication and interpersonal abilities
- Proficiency in Microsoft Office and care management systems
- Strong organisational and problem-solving skills
- Valid driver’s licence and access to a reliable vehicle
- Full Australian working rights
- Current (or willingness to obtain) National Police Clearance
