We are looking to engage highly skilled Complex Case Managers / Clinical Case Managers to deliver trauma-informed case management services. In this role, you will manage a caseload of clients with complex needs, providing high-quality support and facilitating access to health, mental health, and psychosocial services. This is an initial 3-month contract, with potential extensions, based across multiple locations Australia-wide. Flexible working arrangements are also available.
The successful Complex Case Managers / Clinical Case Managers will be responsible for, but not limited to:
- Undertake intake and risk assessments and develop tailored wellbeing plans.
- Provide specialised case management and clinical advice to enhance service delivery.
- Build and maintain strong relationships with internal teams and external stakeholders, including community organisations and service providers.
- Escalate high-risk issues and contribute to high-level client summaries and briefs.
- Act as the primary point of contact for allocated clients, coordinating access to services and supports.
The successful Complex Case Managers / Clinical Case Managers must have:
- Tertiary qualifications in Nursing, Psychology, Occupational Therapy, or Social Work.
- Professional experience working with individuals with complex support needs.
- Strong understanding of disability impacts and capacity-building models.
- Excellent communication skills and proficiency in IT systems.
- Experience in mental health services and familiarity with evidence-based therapeutic interventions will be highly regarded.
