An exciting opportunity exists for a Clinical Nurse to join our sub-acute Transition Care Service in Walkerville on a full time basis. You will provide clinical leadership and expert nursing care to patients with complex and ongoing healthcare needs. Working as part of a multidisciplinary team, you will play a key role in supporting clinical excellence and contributing to positive patient outcomes in a dynamic and rewarding healthcare environment.

Your opportunity

  • Coordinate patient care and support smooth transitions across care settings.
  • Delivery high‑quality, evidence‑based nursing care.
  • Supporting governance, risk management, and service coordination.
  • Contributing to operational processes and system improvement.
  • Promoting consistent, safe, and effective clinical practice across programs.
  • Participate in quality improvement, service development, and multidisciplinary collaboration.
  • Provide clinical leadership and act as a resource for nursing teams.

About you 

  • Registered Nurse with current unrestricted AHPRA registration. 
  • Minimum 3 years post registration experience.
  • Experience or interest in governance, service coordination, or operational management.
  • Advanced clinical decision making and critical analysis skills.
  • Strong organisational and leadership capability.
  • Ability to manage rosters, budgets and resources.
  • Commitment to patient‑centred care and continuous improvement.
  • Hold current First Aid and CPR certificates.
  • Ability to participate in a rotating roster.
  • Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
  • If successful, willing and able to provide proof of up-to-date vaccinations.
