About the role

Wintringham’s Williamstown Hostel is seeking an experienced Care Evaluator to join our team. Bring your expertise in resident-centered care to our close-knit community, providing clinical guidance, care planning and hands-on support while working alongside residents and care staff to promote independence, dignity and quality of life. Reporting to the Residential Site manager, responsibilities include:

  • Deliver high-quality, person-centred care that promotes residents’ independence, dignity and lifestyle choices.
  • Assess residents’ needs and develop, review and update Support Plans in partnership with residents and their care teams.
  • Build a strong understanding of each resident’s care needs through hands-on care and close collaboration with care staff.
  • Monitor changes in residents’ health, medication, behaviour and lifestyle, developing appropriate strategies to respond to changing needs.
  • Provide clinical guidance and support to care staff, working collaboratively to maintain high standards of care and achieve positive outcomes for residents.

What are we looking for?

  • Experience in gerontology / community / palliative / mental health / alcohol and drugs or related specialty
  • Registered Nurse Division 1
  • Well-developed assessment and documentation skills
  • Knowledge and understanding of the special needs of older people 
  • Ability to liaise with staff, residents, their families and other professionals 

What we offer

Employment type: Part Time Role

Pay: $53.21/hour – $62.84/hour (depending on your quals & experience) plus super. Above Award pay and not-for-profit salary packaging benefits of up to $18,500 each year.

Location: Williamstown (VIC)

Roster/hours of work: 6 shifts per fortnight, including a required weekly Wednesday shift, with flexibility around the remaining working days. Shifts are either (0800 – 1600) or (0900 – 1700)

Week 1: Wednesday + 2 agreed days

Week 2: Wednesday + 2 agreed days
